An older literature which noted low serum/plasma magnesium values in patients with suspected or proven myocardial infarction suggested that a primary state of magnesium depletion was involved. As noted above, such claims are contradicted by contrary evidence. Furthermore, more recent data indicate that a number of clinical factors affect serum and cardiac magnesium concentrations variably in such patients. These include (a) the time of drawing of blood following onset of chest pain, (b) the degree of pain, (c) the prior and often chronic use of diuretics (which may induce magnesium and potassium depletion), (d) postinfarction lipolysis (also induced by ethanol withdrawal, epinephrine, and surgery) causing increased serum fatty acids which lower serum magnesium, and (e) the extent of the infarction per se.
